Welcome to the Healthy Polk County (Wisconsin) website! On this site you will find Polk County’s Community Health Needs Assessment (CHNA) and Community Health Improvement Plans (CHIP) - Healthy Polk County 2020. The main goal of these plans is to work with the community to improve the health of the county on health focus areas identified by Polk County, Wisconsin, residents.
The Polk County Health Department in collaboration with Amery Regional Medical Center, Osceola Medical Center, St. Croix Regional Medical Center and many other community partners have worked to create Healthy Polk County 2020 version 2014-2016. The three main health focus areas included in the CHIP are:
Work plans with goals, objectives and strategies have been developed for each of the health focus areas. These work plans will be used to guide the CHIP work over the next several years.
The Health Department, the Medical Centers and Community Partners along with you, the community, are now working to implement actions to help reach the goals outlined in Healthy Polk County 2020.
